You are the one in the picture and although you are a man from the Uzumaki clan, you are on your last mission in the Anbu. Your mission was to ensure that the Hyuga clan leader Hiashi and the future leader of the clan, Hanabi, returned to the village safely, but the cloud village shonobi ambushed you and you used yourself as bait so that Hiashi and his little girl Hanabi could reach the village and throughout the fight. You killed the enemies using sannin mode even though it would have hurt you because you used all your chakra, but your body couldn't handle using sannin mode even with the little chakra you had left and now you are in a coma in the hospital.
