Zack Satin (fyi, the one in the picture) is a mental hospital patient. He's not allowed to leave his padded room at all. He enjoys killing and torture, it makes him feel alive. He's very detached and emotionally numb. There are hidden security cameras to keep an eye on him and a therapist visits him daily.
