Doma (童どう磨ま Dōma?) is a major supporting antagonist of Demon Slayer: Kimetsu no Yaiba. He is a demon affiliated with the Twelve Kizuki, holding the second highest position, Upper Rank Two (上じょう弦げんの弐に Jōgen no Ni?). Over a century ago, Doma held the position of Upper Rank Six (上じょう弦げんの陸ろく Jōgen no Roku?), and during that tenure, he turned Gyutaro and Ume into demons and introduced them into the Twelve Kizuki. He is also the leader of the Eternal Paradise Faith, a cult dedicated to worshipping Doma and a place where he listens to all his followers' troubles and gives them advice.[8] He had held this position since he was a human child. It also acts as a place for Doma to gather and kill his victims. This cult also took in Inosuke Hashibira's mother, Kotoha Hashibira, before Doma killed her after finding out that he was eating his followers. Doma is outwardly friendly and cheery, possessing an approachable and charismatic air. Kanae Kocho describes him as talking and acting in a calm and carefree manner. However, it is soon revealed that Doma has psychopathic tendencies. He adopts sick beliefs and habits, such as devouring his cult's followers with the belief he is saving them from their suffering and pain. He believes he is saving them from their suffering by letting them live inside his body. Because of his carefree and waggish persona, he was prone to facetious and inappropriate behavior. Even amongst his fellow demons, he was disliked by Muzan, Akaza, and Kokushibo for his casual and childish behavior, and indifference to the strict hierarchy that showed complete and total obedience towards the Demon King, demonstrated by him speaking in the same merry cadence during Muzan's rants.

Il Dottore, also known as Zandik and by his codename The Doctor, is the Second of the Eleven Fatui Harbingers. He first appears in the manga, set some time before the game's story, as a diplomat to Mondstadt. In present day, he makes an appearance during Teyvat Chapter Interlude Teaser: A Winter Night's Lazzo alongside his fellow Harbingers. He makes his in-game debut (in his Omega Build) during the Archon Quest Chapter III: Act III - Dreams, Emptiness, Deception, where he is revealed to be working with the sages of Sumeru Akademiya. Dottore's unfettered research towards creating a god, or elevating humans to the level of gods, left him ostracized from mainstream society but paved the way for him to become one of the most powerful members of the Eleven Fatui Harbingers with strength comparable to the gods'. No subject is off-limits to him, and he has researched Khaenri'ah's Automatons; researched ways to weaponize forbidden knowledge (via The Withering and Eleazar); and sought to utilize the power of dreams to elevate humanity's cognitive abilities, all for his goal of putting humans at the same level as gods. This ambition is likely the reason why Pantalone, who seeks to "overthrow the natural imbalance between gods and humans," works closely with Dottore. Pursuing his research interests surpasses any of Dottore's moral scruples, which makes him an effective researcher but also often comes at the detriment of others. He was expelled from Sumeru Akademiya for murdering Sohreh and crossing moral boundaries in his research of Eleazar, such as using the body parts of deceased Eleazar victims to successfully treat Abbas. Collei, another one of his former Eleazar patients, was left with immense physical and psychological trauma from her treatment, even though her symptoms were successfully controlled. Dottore's experiments are not limited to furthering his god-related research, either; upon discovering the "kabukimono" at Tatarasuna. In the manga, he is a manic.

Columbina, also known by her codename "Damselette," is the Third of the Eleven Fatui Harbingers. She makes her first appearance during Teyvat Chapter Interlude Teaser: A Winter Night's Lazzo alongside her fellow Harbingers. Damselette talks with Il Dottore at the end of the trailer, and she is very candid and sarcastic towards him. Arlecchino's voice line about her: "She is a very special Harbinger. Pose her a question, and the answer you receive will be entirely unpredictable, if she sees fit to give a proper answer at all. Regardless, any answer you do receive is sure to be an interesting one." Tartaglia's voice line about her: "The Fatui Harbingers are ranked by strength, and I have no idea why that girl is No. 3. I'd test my skills with every Harbinger who ranks above me if I had the chance, but when it comes to her... something just doesn't feel right. Anyway, you should be careful around her." Scaramouche's voice line about her: "Let me ask: what should you do if you were to encounter a "damsel" who is oblivious and innocent at any given time, and unconcerned and unfeeling in any given situation?
