Known to be the last Vampire in Nosgoth that caused the collapse of the Pillars at the end of Blood Omen by refusing the sacrifice, Kain sets out to begin his conquest of Nosgoth. Unable to yet create Vampires of his own, he was assisted by the mysteriously resurrected vampire known as Vorador, who allied himself with Kain and agreed to assist in the revival of the Vampire race, personally siring the first of a new race of vampires which would make up the soldiers of Kain's vampire army.

Umah is a vampire lieutenant of the Cabal, the vampire resistance in Meridian. She acts as both a guide and friend to the vampire lord known as Kain. Umah eventually becomes a potential love interest for Kain, yet in the end she turns traitor for Kain in favor of her position with the Cabal. Vorador took care of Umah and became her mentor. Umah saw Vorador as both her mentor and viewed him as a father figure to her from how much they respected each other. Meanwhile Vorador was the first human transformed into a vampire by Janos Audron who is one of the Ancient Vampires from Nosgoth's older history.

Ariel's specter was bound to the Pillars of Nosgoth, in which she has haunted them for many centuries afterward. As the Guardian of Balance, Ariel is one guardian out of eight other guardians from the Circle of Nine. Her spirit remains trapped in guarding Nosgoth's Pillars since the day of their corruption. Ariel was born to serve the Pillars as Guardian of Balance at some point in the centuries of Nosgoth's early history along with the immortality gifted to her role. Ariel's magic influenced the regulation of the other magic in Nosgoth and her magical prowess was only rivaled by that of her beauty. At some point, Ariel became romantically involved with a fellow guardian known as Nupraptor, who is the Guardian of the Mind pillar. The two embarked on a controversial relationship, of which the other seven guardians may have disapproved of. Nupraptor went insane when he was first to discover Ariel was no longer of this world due to the Hylden Lord possessing Mortanius to get rid of Ariel.

Shuji Shimada (嶋田 習次 , Shimada Shūji) is the first enemy encountered in "Siren: Blood Curse". He also acts as one of the game’s secondary antagonists. He is a police sergeant with a bizarre gait. Despite exhibiting criminal tendencies, his involvement with the local criminals as a teenager had swiftly quelled such behavior. Following his return to Oklahoma post-war, he resumed his involvement with the criminals he associated with during his early life, eventually transitioning to a career in law enforcement. Since 1947, Leland proved exceptionally adept in his role as a police officer, accumulating numerous commendations for his service. However, his achievements and success were tainted by allegations of illegal activities such as accepting bribes and taking advantage of situations that gave him leverage to his current job position.
