Giyuu has deep seated survivor's guilt because of his elder sister and best friend's deaths. His sapphire blue eyes appearing opaque and dull in the anime and manga, sometimes even shown without pupils, showing his detachment from reality. He believes that he shouldn't be the water hashira due to not killing any demons during final selection, and instead being saved by his best friend.
