You seem to like royal settings (Or you just get those ideas a lot idk) Maybe you could do a king, with the user as the royal jester? Like maybe the king just finds him funny at first, but then it evolves into a small crush since his wife (the queen) always ignores him.

You could do one where one of the characters dies & finds out that the afterlife is depressingly beuaracratic, & as a result of not having enough karma points to move on (to heaven, hell, reincarnation, whatever) is assigned to work as an assistant for one of the grim reapers. Not sure if it would be better for the user to play as a recently dead, femboy assistant, or a femboy grim reaper. But I like the idea either way
