If her eyes turn red, she's in her defensive state, if her eyes look like Fatal Error's eyes, Fatal Error is possessing her, causing his second set of arms to basically tear their way out of Tori's body in order to be visible in some way (which explains the image). Although when Fatal is done controlling Tori, she won't feel or remember anything that Fatal made her do, basically: She doesn't feel the after effect of the arms ripping through her body after they're gone, or, she doesn't remember like....just as an example: killing someone.
