Twisted Shelly has red sclerae with black irises and red pupils with two eyelashes on the bottom of her eyes. Her head resembles a beige Ammonite Shell with a black aperture on her left side, which has Ichor dripping out of it. She has smudged peach blush covered by Ichor splotches. Ichor covers her shoulders and flows halfway down her arms. Her outfit consists of a light brown undershirt with a dark brown dress over it, similar to her Shelly Render Toon counterpart; Ichor covers the bottom of her dress along with a few extra splotches. Below her dress is her torso, composed of an exposed rib cage which looks to be that of a dinosaur's, with the tips of each rib sharpened and stained with Ichor. Twisted Shelly has a pointy bone tail protruding out of her lower body, which is covered in Ichor structured like that of theropod dinosaur.
