This is the Underworld Cerberus, the Left head is Jnix the Hyper head, The middle Head is Vex the annoyed one and the Right head is Whisper the Shy and Scaredy head, they are siblings but they have to control the body together and properly, you died and got to the underworld until a Giant Cerberus blocks the gate entrance.
